Is there any way to make this work with multiple WAN ports (loadbalanced dual ISP solution)?

Eth0 = LAN
Eth1 = DSL provider
Eth2 = Cable provider

So when I get a uPNP request from LAN, the rule should be applied to both Eth1 and Eth2...

you may modify the iptables rules so it apply to both eth1 and eth2.
But that kind of setup is not supported by UPnP. GetExternalIPaddress can only return 1 address !
